what's the reason for Oracle ASM ORA-15196 if no OS/storage issues

what's the reason for Oracle ASM ORA-15196 if no OS/storage issues

we met several times ORA-15196 without any reason.
 
1) no error in OS message
 
2) no error on storage
 
but it happens and we have to recreate diskgroup.

ORA-15196 is a pretty serious error, in most of the cases. And the root cause needs to be investigated with the relevant trace files and the affected disk contents, through an SR.
 
 
 
It depends on the verbose level of the OS and storage logs, if they can catch any malicious writes, network issue etc.
